Step 1: Click on Individual/Retailers
Step 2: Login
Step 3: Username – Your UHID is your username.
Step 4: Password – Your date of birth (in DD/MM/YYYY format) is your password.

For Retail Health Product

Step 1: Click on Employee/Ecard
Step 2: Login
Step 3: Username – Empid@Corporatename (e.g., if employee ID is 12345 and corporate name is Zurich Kotak General Insurance, then user ID would be 12345@KMGI)
Step 4: Password – Your date of birth (in DD/MM/YYYY format) or password sent to your official email at the time of enrollment. You can also click on "Forgot Password" to reset.

For Group Health Product (Serviced by FHPL)

Step 1: Visit – https://www.paramounttpa.com/PolicyClaim/PolyClaimStatus.aspx
Step 2: Select Insurance Company and Policy Type from the dropdown menu
Step 3: Select either PHS ID or Employee ID and enter details accordingly
Step 4: Choose IPD and click Submit

For Group Health Indemnity (GHI) claims

Step 1: Visit – https://kotakgmc.paramounttpa.com/Login_policy.aspx
Step 2: Enter Certificate Number
Step 3: Enter DOB in DD/MM/YYYY format of policy owner/proposer
Step 4: Click on Login

For Hospital Daily Cash (HDC) claims

Step 1: Go to https://mdindiaonline.com
Step 2: Click on the "My Account" menu and select one of the following:

For MD INDIA:

Select Insurance Company Name from the dropdown list.
Enter Policy Number OR MDID Number.
Enter the Captcha Code.

Policy Owner (for Retail Users)

Employee ID: Enter your Employee ID.
Password: Enter your Date of Birth (in DDMMYYYY format).
Enter the Captcha Code.
Important Note:
Both logins are OTP-based.
OTP will be sent to the registered email ID and mobile number.
Enter the OTP to proceed.

Corporate Employee (for Corporate Employees)

Step 1: Login URL – portal.mediassist.in
Step 2:
Username – Your username (provided by MediAssist)
Password – Your date of birth (in DD/MM/YYYY format)
Step 3: Click on Policy → Download E-cards

For MediAssist:

Step 1: Click on the login link – https://www.healthindiatpa.com/Logins/LoginCorporateEmployee.aspx
Step 2: Login credentials
Username: @ (e.g., if Employee Code is 12345 and Group Code is ABCD, then user ID would be 12345@ABCD)
Password: or click on "Forgot Password" to get the password on your registered email ID.
Enter CAPTCHA and click on the login button.

For HealthIndia TPA:

Step 1: Login to https://wallet.volohealthtpa.com/
Step 2: Enter your login credentials
Username: XYZ
Password: ABCD
Step 3: Once you submit the credentials, you will be prompted to reset the default password (mandatory).
Network Hospital Link: https://wallet.volohealthtpa.com/public/hospital-network
Step 4: Health Policy page displays your e-cards, claims details, and the list of network hospitals across India.

You can now access your health insurance policy information anytime and from anywhere.
