How to track your health insurance claims status?

For FHPL:

Step 1: Login– https://www.fhpl.net

Step 2: Click on Logins on the home page.

Step3 : 

For Retail Health Product 

  • Click on Individual/ Retailers

  • Login

    • Username – Your UHID is your username.

    • Password – Your date of birth (in DD/MM/YYYY format) is your password.

For Group Health Product (Serviced by FHPL)

  • Click on Employee/ Ecard

  • Login

    • Username – Empid@Corporatename (e.g if employee ID is 12345 and corporate name is Zurich Kotak General Insurance. Then user ID would 12345@KMGI)

    • Password – Your date of birth (in DD/MM/YYYY format) or password sent to your official at the time of Enrollment or click on forgot password to reset.

For Paramount:

For Group Health Indemnity (GHI) claims

Step 1: Visit– https://www.paramounttpa.com/PolicyClaim/PolyClaimStatus.aspx

Step 2: Select Insurance Company and Policy Type from drop down menu

Step 3: Select either PHS Id or Employee ID and enter details accordingly

Step 4: Choose IPD and Click Submit

For Hospital Daily Cash (HDC) claims

Step 1: Visit– https://kotakgmc.paramounttpa.com/Login_policy.aspx

Step 2: Enter Certificate Number

Step 3: Enter DOB in DD/MM/YYYY of policy owner/proposer

Step 4: Click on Login
